• Parents or guardians who claim Social Security as their only source of income MUST submit the SS Only Statement along with the current benefits statement.

• Parents or guardians who are unemployed MUST submit a benefits claim statement from LaWorks along with any other documents from the list below that may apply to their current financial situation.

• Parents or guardians who are employed intermittently, self-employed, or who don’t have check stubs or benefits print outs to verify income, MUST submit a Declaration of Income for Irregular Employment Form along with a Statement of Support Form if applicable.

• Parents or guardians who claim zero income of any kind MUST submit a Statement of No Income Form along with their spouse’s allowable income documents or Statement of Support Form, whichever is applicable.
